Position description:
The Social Worker directs, supervises, evaluates, and provides medical social services to clients in the home as prescribed by the attending physician. The Social Worker functions under the direction of the Clinical Nursing Supervisor and cooperates with the primary care nurse in the coordination of care of the client.
Position purpose:
Comprehensive evaluation of psychosocial status as related to the client’s illness and environment and development of a social work care plan.
Assist physician and other team members in understanding significant social and emotional factors related to health problems.
Participation in case conferences with the health care team.
Participation in the development of the total client care plan.
Works with the family.
Identification of client and family needs for other home health services.
Maintain compliance with all agency policies and procedures.
Physical requirements:
Regular exposure to client elements with some general office.
