Position description: 
Registered Nurses/Case Managers are assigned primary responsibility for the coordination of the patient's care; providing skilled nursing care, teaching and supervising the family, and other members of the nursing team. In addition, maintenance of all documentation pertinent to the patient's record is required. These services are performed in accordance with the physician's orders and under the supervision of the Director of Clinical Services and the Supervisor of Clinical Services.
Position purpose: 
-Accepts patients assigned by the Supervisor of Clinical Services and conferences with the supervisor/designee regarding patient/family needs, Plan of Care, and goals. -Implements and coordinates the Plan of Care and services for the patient. -Initiates preventative and rehabilitative nursing measures as appropriate for the patient's care and safety. -Observes and records the patient's signs and symptoms and reports all changes in physical and mental condition to the physician and Nursing Supervisor -Case conferences with all disciplines involved in care at least every two (2) weeks. -Confers with physician regarding patient status as necessary, but no less frequently than every sixty (60) days -Obtain orders for any necessary changes to the plan of care, medications, and/or visit frequency including recertification/episode renewal orders -Provide patient/family with necessary education/information -Teaches and supervises the patient and caregivers -Participates in case conferences, inservices and other meetings as required. -Performs or provides for supervision of home health aides at required time frames.
Special skills: 
Bilingual a plus! Moderate to advanced computer skills
Physical requirements: 
Able to lift, turn and transfer clients weighing up to 200 pounds. -Must be able to stoop and bend. -Visual/hearing ability sufficient to comprehend written and verbal communication. -Provide direct care according to the established patient plan of care. -Delegate care by other staff. -Carry out a wide variety of complete and involved treatments, procedures, and examinations. -Must be able to handle stress and remain calm under pressure.
